Survey Binder, a service of Audrow Nash LLC ("we"), provides survey-readiness services to home care agencies. This policy explains what we collect and how we use it.
What we collect
- Contact and business details: your name, email, phone, agency name, and license or registration number.
- Payment details: processed by Stripe. We never see or store your full card or bank numbers.
- Readiness tracking data: for the monthly plan, due dates and status (for example "staff member J.S.: training due Nov 2026"). We don't collect Social Security numbers, dates of birth, background-check results, or copies of personnel documents.
- Public records: licensing information that the State of Florida publishes about agencies.
- Basic website analytics: such as pages visited, which may be collected by our hosting provider.
What we never collect
We do not request, accept, or store client or patient health information. If you send us any by mistake, we'll delete it and let you know.
How we use it
- To deliver the services you buy, send reminders, and support you.
- To bill you and keep records we're legally required to keep.
- To contact agencies about our services, using public licensing records. You can opt out anytime.
We don't sell your information.
Who we share it with
Only service providers that help us operate: Stripe (payments), Cloudflare (website hosting and email routing), and Google Workspace (email and documents). They process data on our behalf.
Retention and deletion
We keep your data while you're a customer. If you cancel, we'll send you an export of your tracking data on request and delete ours within 30 days, except billing records we must keep by law. To request access or deletion, email us.
